Empower Tehama seeks an experienced Case Coordinator/Group Services Facilitator to provide comprehensive case management assessments, develop and monitor individualized case plans, build relationships with community partners to avoid duplication of services and refer clients for needed services including legal, therapy, psychoeducational groups, parenting, etc.
About the Job: Case Coordinator/Groups Services Facilitator will assist Prop 47 and BIP participants with identifying, implementing and achieving goals found through a comprehensive assessment; develops, coordinates, manages, and monitors case plans for participants; refers participants to needed community resources and assist participants with accessing those resources necessary to meet their case plan goals. Coordinates and builds relationships with community partners to avoid duplication of services; assist clients with identifying and addressing barriers to success in obtaining and securing housing; facilitates or co-facilitates groups about relationships, life skills, anger management, etc. Must be able to clearly and accurately document services provided to clients and other duties as assigned.
Must be proficient in Outlook and have working knowledge of WORD and Excel. Must be able to operate a multi-line phone system. Ability to work with a broad range of people and maintain an environment that is conducive to serving high-risk populations and people in crisis. Strong active listening skills, professional phone etiquette, good decision-making, a nonjudgmental and accepting attitude, and attention to detail.
