Case Manager

Under the guidance of the Program Manager, this position functions as a member of the 6th Street Center Team. The 6th Street Center Case Manager is a key position in providing care to the youth who enter center services. This individual will complete intakes, write service plans, contribute to staff meetings and ensure all service plans are being followed. This individual must provide services according to the policies and procedures of Youth for Change. Case Managers must be flexible, organized, and possess excellent written and oral communication skills.


  • Conducts and completes the intake of new program participants
  • Provides proactive crisis intervention/prevention techniques and utilizes emergency decision-making as appropriate
  • Provides information and referral to youth and parents/guardians when applicable
  • Act as community, family, and youth liaison
  • Participates in staff training, in-service, and development
  • Completes all documentation and reports as required
  • Contacts parents/guardians whenever appropriate
  • Retains accurate records, timesheets, clinical notes, etc.
  • Maintains professional standards and behavior at all times
  • Maintains cooperative and positive working relationships with the youth and co-workers at all times
  • Provides follow-up with youth who have exited services according to YFC policies and procedures
  • Transports youth in accordance with service plan and YFC policy
  • Conducts home visits to youth in transitional and master lease housing programs
  • Assists in daily center operations including intake and processing of donations, cleaning duties, grocery shopping, and preparation of meals as needed.

  • Minimum education Bachelors of Art in Social Work, Psychology or related field is required
  • Master’s in Social Work (Preferred)
  • Experience working with children and families “at-risk”
  • Knowledge of 24-hour crisis intervention
  • Valid California driver’s license
  • Fingerprints and background checks including Child Abuse index clearance from the Department of Justice
