Overview

1. Provide intensive case management services, mentorship, and positive relationship to severely mentally ill (SMI)
participants, and co-occurring participants (SMI with alcohol/drug addictions) in the Plumas Commons (PC)
supportive transitional housing program
2. Provide referral and accompaniment to PC participant appointments
3. Assist PC participants with basic life skills education, including but not limited to home management, money
management, social skills, healthy hobbies, stress management, employment skills, and personal hygiene
4. Help participants apply for assistance programs and public benefits, such as: Section 8 Housing, Cal-Fresh, Social
Security, IHSS (In-Home Supportive Services)
5. Assist participants apply for/obtain vital documents, such as California ID, SS Card, Birth Certificate, Diplomas,
Transcripts, etc.
6. Monitor PC participants’ medication compliance
7. Make frequent home monitor visits as part of each PC participant’s individual treatment plan
8. Know or learn MediCal billing/ electronic health record system
9. Write clear and thorough case notes regarding PC participant visits within mandated timelines
10. Maintain adherence to projected billing ratio requirements
11. Contribute to ongoing written assessments of PC participants’ progress
12. Transport PC participants when necessary
13. Provide clear communication with County Behavioral Health, community agencies and PC supervisors regarding
participant circumstances
14. Inform PC supervisors of any illegal or harmful (or potentially so) situations among participants
15. Follow all program guidelines regarding client confidentiality and HIPAA compliance
16. Attend orientation, staff meetings, and other trainings as assigned
17. Work under supervision of EA PC Supervisor, Clinical Supervisor, Mental Health Director, and/or Executive
Director
18. The position is hourly and designed to accommodate the schedules of PC participants; hours and days of work are
flexible to satisfy the job requirement
19. Other duties as directed

At least two years’ experience in a social services setting 2. Experience in working with mental health populations preferred

Minimum Months of Experience: 24
Education Required: High School
Driver's License Required: Yes
