Overview

BA or BS degree in social work, psychology, or similar social science or 2 years
experience in outreach and engagement or working with populations in need of services
and supports.

Full Time, Benefited position.

Enhanced Care Management is a whole-person, interdisciplinary approach to care that
addresses the clinical and non-clinical needs of members with the most complex
medical and social needs through systematic coordination of services and
comprehensive care management that is community-based, interdisciplinary, high touch
and person-centered. DHCS’ vision for ECM is to coordinate all care for members who
receive it, including across the physical and behavioral health delivery systems.
These primary duties are to provide targeted case management in addition to identifying
and connecting members to community resources to help overcome barriers to ensure
their needs are met.
Targeted population are adults who are experiencing homelessness or at risk of
homelessness, adults with serious mental illnesses, and or substance abuse challenges.
This position will require travel within Plumas Co, case note documentation, and ECM
billing.

Primary and acute medical care
Dental care
Mental Health services
Substance abuse services
Behavioral services
Extensive job training and vocational
services
Long-term services and supports
Affordable housing
Life skills and education
Supportive housing
Transportation
Accompaniment to critical appointments
