Job Description:

Provides medication management and skilled nursing procedures for residents in compliance with state regulations and scope of practice as defined by state nursing regulatory agency.

Minimum eligibility requirements:

  • Must have compassion for and desire to work with the elderly
  • Must have a current state Nursing License in good standing with the state that you are working in
  • Must have had previous experience working with the elderly
  • Must maintain eight hours of Alzheimer’s and related dementia-specific training per year
  • Ability to read, write and understand English
  • Exceptional organizational skills
  • High level of ability in effective and succinct communication, written and oral with residents, families, staff, vendors and the general public
  • Must be capable of maintaining fulltime attendance
  • Must meet all health requirements, including TB screening
  • Must be able to perform duties and responsibilities (essential job functions), with or without reasonable accommodation
  • Must pass criminal background check
  • Must have valid driver license

Essential functions

Joint decisions & duties between licensed nurse and Wellness Director:

  • Assist with resident assessments as requested by Wellness Director
  • Add information vital to the development and implementation of customized service plans
  • Participate in care conferences as needed
  • Contribute to departmental cost control by using supplies efficiently and avoiding waste
  • Administer medications to residents
  • Receive report from outgoing Licensed Nurse/medication assistant Count narcotics with outgoing nurse/medication assistant
  • Monitor the care of all residents
  • Monitor the performance of caregivers to assure quality care
  • Model courtesy, tact, and professionalism for the team
  • Assure safety, cleanliness and appropriate lighting and temperature of the resident rooms and building
  • Document changes in resident conditions in 24-hour report and resident record, write incident and/or accident reports, notify physician, family and Wellness Director
  • Chart medications in Medication Administration Record, document results of PRN medications
  • Lock outside doors on PM shift; unlock outside doors on AM shift
  • Write physician’s orders when telephoned in and fax to the physician for signature immediately
  • Receive reports from caregivers before end of shift
  • Answer and respond to calls/concerns from family members and physicians
  • Admits and discharge residents (permanent and respite) to the community
  • Reviews MARS after each cycle change
  • Give report to oncoming Licensed Nurse/medication assistant
  • Advise Wellness Director of any change in resident condition, employee issues or family concerns
  • Meet paramedics on-scene with essential information regarding the resident when 911 is activated, notify physician, family member and Wellness Director
  • Take inquiries about the community after hours using the guest registration card; notifies Executive Director or Community Relations Director with the information
  • Give tours of the community in the event the Executive Director or Wellness Director is not available
  • Make a significant contribution to the quality of life of the people who live in the community by initiating meaningful connections and interactions

Supervisory functions

  • Act as a resource to care associates and other staff members while on duty
  • Adjust staffing assignments in the absence of the Wellness Director
  • Participate in daily standup meetings
  • Weekly meeting with Wellness director
  • May act in the capacity of Wellness Director in her or his absence

OSHA category classification II:  Potential for exposure to bloodborne pathogen and requirement of the job change.

Education Level: High School Diploma or Equivalent

Requires a Driver’s License: Yes, Operator License

Minimum Salary: 27.00 Hour  

Maximum Salary: 27.50 Hour

Pay Comments: DOE (Depends on Experience)
