Overview

We are looking for Caregivers to provide professional support services by assisting individuals with developmental disabilities/and or physically challenged, who wish to live in the least restrictive environment as possible in their own homes. We develop a customized care plan for each individual based on their needs, interests, and goals, allowing them to carry out their daily activities, maintain their social contacts, and continue to pursue their hobbies, goals and interests.
The goal of Chance 4 Change is to promote self awareness, self-esteem, self determination and self acceptance within the individuals we serve.

Job duties include assisting individuals with maintaining their homes, daily chores, meal planning/meal preparation, grocery shopping, assistance with medication, attending doctors appointments, transportation and accessing the community.

We are seeking applicants who are compassionate and positive. Who are honest, dependable and flexible. Applicants must be able to pass a background check and must be able to be CPR/First Aid certified. All applicants must have a Drivers license, reliable transportation and auto insurance. Due to COVID-19, Applicants will be trained to follow, teach and implement all safety protocols using protective gear such as gloves, masks and frequent hand washing.
